So intense. And what an awesome visual concept! it makes me cringe but the idea itself is very original, yet very fitting for the theme. Reminds me of an older piece of yours. Same strength in the visual concept.

The texture of the background is awesome (that's the acrylics, if I'm correct?)
The hand is perfectly painted. All the veins and the details... beautiful.

The double shadow on the wall is so real as well. Did you use photo reference?

It's kind of freakish, but I also understand the sentiment somewhat and like it. I'm just thankful you weren't more graphic with the pen - hand thing, otherwise I would just not feel very well at all. The background and the heart are somehow very touching, the texture and the color. I like the way the hand has so much realistic feel to it, but it distracts me a bit that the hand still looks painted but the pen looks more like photorealistic. I also like the dynamic feel that the shadow creates.
